Appointment Terms and Conditions
The Client hereby acknowledges and agrees that a credit card is required to reserve any appointment. For non-group appointments, a minimum of twenty-four (24) hours notice is required for cancellation or rescheduling. A fee of $100 will be charged if an appointment is canceled or rescheduled less than twenty-four (24) hours in advance or in the event of a no-show.
For group appointments, a seventy-two (72) hour cancellation policy applies, and the credit card on file will be charged $200 if canceled less than seventy-two (72) hours in advance. Clients arriving more than ten (10) minutes late for any appointment may be required to reschedule to avoid disrupting other clients' appointments.
Service and Payment Terms
Consultations
The Company provides consultations and in-depth skin analysis with expert providers. Consultations include education about the Client's unique skin type, a comprehensive discussion about all available options, both immediate and long term, and a clear plan of action based on individual goals.
Pricing and Payment
Pricing for products and services is subject to change at any time without notice. Prepaid services must be redeemed within one (1) year of purchase. In the event of any payment issues with a client's credit or debit card or CareCredit, including but not limited to chargebacks or fraud issues, the Company reserves the right to require cash payment for all future transactions.
Client Policies
Clients under the age of eighteen (18) must be accompanied by an adult when receiving services. To ensure the safety of children and the enjoyment of all clients, parents or guardians must make alternative arrangements for children while receiving services.
No pets are permitted on the premises, with the exception of working service animals.
Indemnification
The Client agrees to defend, indemnify and hold harmless the Company, its affiliates, subsidiaries, and each of its and their respective directors, officers, shareholders, managers, employees, agents, partners, representatives, licensors, successors, and assigns (collectively, "Related Parties") from any claim, demand, loss, award of damages, expense, or cost (including reasonable attorneys' fees) that arises out of:
(a) Use of, inability to use or access, or reliance on, the Platform or the Services;
(b) Provision or receipt of a Treatment;
(c) Travel in connection with the provision or receipt of a Treatment;
(d) Interaction with any other User;
(e) Violation of any provision, term, condition, covenant, warranty, or representation in this Agreement;
(f) Violation of this Agreement by the Client or any other person using the Client's User Account;
(g) Acts or omissions resulting in harm to person or property;
(h) The Company's use, disclosure, or preservation of User Content;
(i) Violation of any rights of another, including intellectual property rights.
Modifications
The Company reserves the right to modify these terms at any time. Continued use of services following any such modifications constitutes acceptance of the modified terms.
